A First Counselor is one who serves as a member of a presidency, priesthood quorum, or bishopric. A First Counselor has specific responsibilities, and serves with a Second Counselor and often a secretary. The First Counselor is also available to assist the president (or bishop) when needed. The Church organizations are similarly organized.
